Provide comprehensive care to families as well as engage in health promotion activities. We have multiple opportunities for both full time and part-time NP's.
Job Duties and Responsibilities:
- Evaluate and manage acute and chronic medical conditions
- Provide initial assessment and management of mental health conditions e.g. depression and anxiety disorders
- Order and analyze necessary diagnostic tests using high value care principles and EBM to inform clinical decision-making
- Prescribe appropriate treatment and/or medication in an evidence-based manner and within the scope of licensure
- Advise patients on treatment recommendations, lifestyle modifications, and age-appropriate preventive screenings
- Refer patients for consultant services when necessary
Additional responsibilities will include:
- Health promotion activities and recruitment efforts for the site
- Maintaining accurate and detailed medical records about each patient, including observations during examinations
- Completing EMR documentation, billing, and patient-related paperwork in a timely manner
- Working with diverse populations including recent immigrants to the United States
- Participating in night and weekend call shared among clinicians
- Collaborating with multidisciplinary team
- Attending and participating in clinic, provider, and all staff meetings
- Participating in pre-visit planning activities with the other members of the care team Participating in center wide and clinic-specific quality improvement projects and peer review
- Participating in appropriate certification/maintenance of certification and continuing education activities
Qualifications:
- Nurse Practitioner, Master’s degree required
- Successful completion of an accredited nurse practitioner program
- Certification as a Family Nurse Practitioner
- Current and valid NYS license
- Current DEA certificate
- Updated BLS and infection control certificate
- Excellent bedside manner
- Proficiency in Spanish or an Asian language is preferred
- AAHIVM certification and/or LGBT health certification is a plus
